Health Care Management

Chronic Care Management is a service paid for by Medicare or Managed Care Medicare Replacement Plans that involves building and implementing a patient-centered care plan that is uniquely built for your chronic health conditions and the ways that your doctor, yourself(or rep), and the CCM nurse has decided will benefit you and lead to progressive improvements in your health profile.
You, the patient, are at the center of the planning. We do not follow generalized care plans. Our care plans are built with you (or representative) and take into consideration your wishes on how to carry out the plan
The nurse and you will work together to ensure that the right steps are put into place to meet the agreed upon goals but done at your pace and with your input in regards to frequency and time involved per session to meet your goals.
We will revisit the care plan with each new visit, first going over last session together. The parts of the care plan that are improving and what is not improving and why that is the case.
You will get the results of any testing we do and we will clearly discuss with you why the plan isn't working-if ever it shouldn't. We keep you involved every step of the way.
